Insurance cards:

You need to bring your insurance information at the time of your visit so it can be accurately entered into our computer system. If you do not have your information, we will happily reschedule your visit or you may opt to pay in cash for services rendered on that day.

Appointments:

Tardiness:

Please arrive 10-15 minutes prior to your appointed time to go through the check-in process. If you are 15 or more minutes late for your appointment, we will reschedule you at a more convenient time or you may choose to wait to be seen at a time that we can work you into the existing schedule in order to be fair to other patients.

No-shows:

Appointments will be marked as "no-show" if you do not keep your appointment. If there is a pattern of no-shows, we will warn you of upcoming consequences so you can take appropriate action. We feel that it is important for you care to come in for visits, and if you cannot, that the time be make available for others at the soonest instance you realize you are not able to come into the office.

Cancellations:

We require a 24 hour notice of cancellation. If you cancel your appointment within three hours of your appointment, it will be marked as a no-show because it is difficult for that same office visit to be opened up for another patient in that time-frame (i.e. a call at 8 a.m. to cancel a 10 a.m. appointment - the 10 a.m. time is hard to offer to another patient so that they have enough time for preparation and travel — the initial patient will have a no-show marked in their chart.

Payment:

All co-pays/deductibles are due prior to seeing the physicians. This is a contracted agreement between the physician and your insurance company. If you do not have your co-pay at the time of service, we will ask that you reschedule your appointment so that we are all in compliance with the law. We accept all major credit cards, checks and cash. Thank you.

Medication:

Refills:

Medicines will not be refilled after hours. Please bring a current list with you to your office visit. If you need a refill between visits, please ask your pharmacy to fax a request so we can quickly respond to your need.

Scheduled Medication:

(Nerve and Pain medicine) These types of medicines will not be prescribed on first visits. Dr. Ayo does not practice Pain Management as her primary focus in her practice. We do require Pain Management Contracts if these medicines are ever prescribed to you, and also implement random urine drug screens and pill counts.
